Clinical Judgement and Critical Thinking

Clinical judgement is the ability for nurses to provide appropriate nursing skills and interventions to patients. As stated by Lasater & Nielsen, (2009), clinical judgement is the student’s assessment of their own values, biases, and experiences, and how it affects individual thinking about a patient situation. Critical thinking is a skill in which nurses develop over time when caring for patients which involves according to Sullivan, (2012), an interactive, reflective, reasoning process. Critical thinking comes with experience in the nursing and is important in high risk nursing situations. It is important as nurse educators to assist student nurses with developing critical thinking and clinical judgement skills. Paul’s critical thinking model can be used by educators to assist with teaching and evaluating critical thinking in practice. According to Sullivan, (2012), Paul’s Model for Critical Thinking is a prominent critical thinking model and has been used to improve critical thinking for all levels of students. It focus on three aspects of thinking which are reasoning, intellectual standards and traits. There are various teaching strategies which can assist with the development of the skills.

Simulation is a effective teaching strategy which can help develop clinical judgement and critical thinking skills. Simulation education provides learners with practice with developing processes associated with patient care. For example, learners are given a scenario and they use their clinical judgement to provide appropriate patient care, communication, teamwork, and as the scenario changes or increases to a higher risk situation, the learners are able to utilize their critical thinking skills to continue providing safe effective patient care. Finally, a debriefing is done to review and evaluate the learner’s while assessing their skills and developing new skills. As stated by Sullivan, (2012), to effectively prepare nurses for a future in healthcare they need to recognize not only the value in critical thinking, but also be provided with the time and mentorship to practice this lifelong skill.

Critical thinking and Clinical judgment are two very important parts of the health care world. When a nurse is assessing a patient that is not doing well we need to use critical thinking in order to put the pieces of the puzzle together to help determine what we think might be going on. Once we have gathered out information we are making contact with the provider to update them on the patients condition and we want them to use their Critical thinking while working with the patient to make sure they are able to provide the patients with the care they need. According to Edel (2011) “Critical thinking is the process for reaching a decision that leads to an optimal patient outcome. Critical thinking is a proactive approach to evidence-based practice, and it is necessary for success in today’s challenging health care setting.” Critical thinking is a skill that over time new nurses will improve upon and be able to work through when taking care of very sick patients. In another article by Kienle (2011) “Clinical judgment is a central element of the medical profession, essential for the performance of the doctor, and potentially generating information also for other clinicians and for scientists and health care managers. The recently renewed interest in clinical judgment is primarily engaged with its role in communication, diagnosis and decision making.” When the medical team is together and we are working to diagnose patients who come together it is very important that clincial judgment is used after we get all the information we can to work to start the treatment process or procedures that the patient may need.
